Yes, please call our Toledo, Ohio office at 419-479-5757 to make an appointment.
What Should I Bring With Me For My First Appointment?
Please arrive at least 15 minutes prior to your appointment time if you are visiting our office for the first time. On your first visit, you should bring your insurance card, list of medications that you are taking, and your physician’s referral if required by your insurance company. We also request that you bring all x-rays, CAT scan images and MRI images with the reports that you have had done.
Do You Accept Workers Compensation Claims?
Yes, our office does accept worker compensation claims. If you have a work related injury, please inform us of your claim number, your employer name, the date of injury, your manage care office information and your allowable diagnosis.
Can I Make an Appointment If I Do Not Have Insurance?
Yes, our office will make an appointment for you to see one of our physicians if you do not have current medical insurance coverage. However, please contact our office prior to your appointment date to set up payment arrangements for the appointment or services provided.
When Is My Co-Payment Due?
All insurance co-payments are due at the time of your office visit.
Is My Information and Health Condition Kept Private?
Yes, our office will provide you with a Notice of Privacy when you come to your appointment. The Notice of Privacy will inform you of what we may and may not do with the information that is obtained from you.
I Have An Outpatient Surgery Procedure Scheduled With One Of Your Physicians. What Should I Know?
We will provide you with a Patient Guide to Outpatient Surgery prior to your procedure. You should review the information on the guide on the day of your procedure. You should also refer to this guide frequently on the days leading up to your procedure so you know what to avoid before surgery.
Will I Have To Go To Another Facility for Testing or Other Procedures?
Many of the services that you will need are provided on location. The Toledo Clinic offers radiology, physical therapy, outpatient surgery, an onsite pharmacy and laboratory services. If you do need to go to another facility for testing or a procedure, Dr Smith and Dr DeGere are both affiliated with several hospitals in the area. Our office will work with you and your insurance to find the services and testing that best fit your needs.
Does Your Office Specialize in Certain Areas of Podiatry?
Yes, Dr Smith and Dr DeGere specialize in foot surgery, ankle reconstruction, wound care and issues relating to diabetes. Both of our doctors hold certifications in foot surgery and reconstructive rearfoot and ankle surgery. Both certifications are from the ABPS (American Board of Podiatric Surgery).
